Volunteer information and Forms

 

  • Megan’s Law Background Check:  each volunteer must have a current Megan’s Law Background Check on file. Once the form is submitted to the office with your identification documents being verified by the office it will only be to the district office for processing only after receipt of the following 3 items. Processing may take 7 to 10 working days.
  • Volunteer Code of Conduct:  this form must be signed and returned to the office.
  • Volunteer Code of Confidentiality:  this form must be signed and returned to the office.
  • The law requires that TB clearance (TB skin test or TB Risk Assessment Questionnaire - see below.) be completed by a licensed health care provider.
